REPORTING A DEATH

Inform Alamien Gaidien on 083-752-7175 who’ll see to the R1,000 cash payment to the nominated beneficiary and payment to Paarl Muslim Jamaa for grave and Kafan if the account is in good standing.
If the death occurred at home, the family should secure a doctor to issue the Notice of Death form. The Notice of Death form has 4 copies. Ensure that all four copies are obtained from the doctor and that the doctor does the fingerprinting of the deceased. The form numbered 2 of 3 at the bottom of the page is filled out with the details of the informant together with his/her fingerprints.
If the death occurred in hospital, the doctor on duty will issue the Notice of Death. Ensure that all four copies of this form is obtained from the doctor and the fingerprints of the deceased were taken.
At the hospital as well as at Forensics, the Registered Undertaker (Sh Zarier Moerat) is the only one authorised to sign for the release of the body.
If foul play is suspected, inform the South African Police who will come to the house and they will inform Forensics to fetch the body for an autopsy. Forensics will then issue the Notice of Death.
The PMJ office will make two sets of each form and a copy of the deceased’s identity document. These copies are sent with the Mayet to the Maqbarah to be handed to the Municipality staff.
The original of these documents will be handed to the informant in order to obtain the death certificate and burial order. The burial order must be returned to PMJ to be sent to Drakenstein Municipality for invoicing of the grave within 3 days. The Burial Society will settle the costs of the grave and Kafan to Paarl Muslim Jamaa.
